This form is required to compete in a regional tournament. However, this is NOT required when competing in a Class C tournament. Please print out this form, complete it, sign it, and turn it in.
If you or someone from your family is planning on trying out Dragon, participating in an event, or intends to join as a member, this form must be completed in full. You can print it out at home, fill it out, and bring it in to save some time when you arrive for the first time.
